Occupational Therapy Services for Equipment & Modifications

DAKC’s Zerostep service provides comprehensive Occupational Therapy assessments to ensure independence at home and in the work place. The evaluation examines the person, the environment and their abilities. An independent living service plan is created with the Occupational Therapist (OT) and the consumer. The plan may include recommendations for equipment, adaptive techniques, accommodations, home or worksite modifications and referrals to other services in the community. The Certified Occupational Therapy Assistant (COTA) implements the service plan with the consumer. The COTA also provides set up and training on how to safely use adaptive equipment.

For those who qualify, OT services may be funded through grants by the City of Grand Rapids, Kent County or Kent County Senior Millage. DAKC also provides OT evaluations through Medicaid Waiver, Department of Human Services, Michigan Rehab Services, insurance or private pay based on a sliding fee scale.
